The operator shall submit a report to the commissioner within 60 days after completion of geothermal exploration and include the following information:

1. (1) final hole designation and location;
2. (2) a drilling log noting any water table, aquifers, and sale and mineral deposits encountered;
3. (3) methods of cementing and completion, and types of casing and tubing used;
4. (4) complete details of abandonment procedures;
5. (5) any information on drilling difficulties or unusual circumstances encountered which may be helpful in assuring future safety of operations or protection of the environment in the area concerned; and
6. (6) temperature data and logs for each hole surveyed.
