In addition to the information set out under 11 AAC 40.050, an applicant for a processor registration must submit the following:

1. (1) a physical description of the processing facility;
2. (2) a depiction on the map required under 11 AAC 40.050(b)(5) of where industrial hemp will be stored or processed;
3. (3) if available at the time of application, a list of sources of raw industrial hemp;
4. (4) for applicants who intend to extract cannabinoids and terpenoids from hemp, a description of the proposed extraction processes and of safety measures provided to protect employees, agents, and the public from the dangers associated with the extraction process; and
5. (5) a statement of the intended end use or of the disposal of the hemp plant or plant parts not used for processing.
